Physical Examination

Students in Grades 7th-12th registering for Athletics must have a current physical examination on file with the Athletic Department before the student is authorized to participate in practice/tryouts. Physicals are current for 1 year from the date of the last exam.  

If you have a new physical, please upload the file during Step 4 of the registration process.  If you have a current physical on file at school, we will enter your physical date from our records and you will not need to upload a physical during the registration process.  

There are a number of new questions in this form pertaining to the Dominic Murray Sudden Cardiac Arrest Prevention Act which is effective on July 1 of 2022.  For detailed information regarding these family heart risk factors, signs and symptoms, please visit the following New York State Education Department link.
